Advocacy on Viral Hepatitis is definitely set as part of the FOAESP’S agenda


The policy advocacy about issues linked to Viral Hepatitis is now officially set as part of the State of São Paulo Forum on NGO/AIDS (FOAESP). By the end of January, during the annual planning meeting, the NGOs linked to FOAESP registered a document that will guide the actions at social control and policy advocacy on Viral Hepatitis by 2018.

During the meeting, the coordinator of the Secretariat of Health of São Paulo (SES-SP) Programme on Viral Hepatitis (PEHV-SP), Sirlene Caminada, reported the updated data on Viral Hepatitis in the state. Among the HIV- infected population, the co-infection with hepatitis C is 14,11% and the co-infection with hepatitis B, 9%.

Among the issues reported, Caminada reinforced the need to extend diagnosis during prenatal care and to stimulate hepatitis B immunization, especially among vulnerable population. Still according to the PEHV-SP coordinator, it is necessary to rearrange the logistic of the treatment in order to extend and democratize the access, regardless the patient fibrosis stage level, aiming to ensure the supply of medicines for hepatitis C. At the public services, the actions on prevention must encourage testing, the confirmatory diagnosis by PCR, the treatment of the co-infection as well as introduce the concept of cure after appropriate treatment.

In the FOAESP’S Guidelines on Policy Actions it was listed the following priorities:
-  Stimulate Hepatitis A and B immunization coverage as well as hepatitis C testing, specially for individuals aged 40 and older.
- Propose and monitor the creation of the Technical Chamber on Viral Hepatitis in the state.
- Due to the Hepatitis A outbreak in the State, it arises the necessity of extending the information and referrals, first and foremost among the more affected and vulnerable populations (as MSM and others).
- To ensure hepatitis C periodical testing among the PLWHA, as determined by the protocol.

The funding for the activities
The Parliamentary Front Against STD/HIV/AIDS at the Legislative Assembly of São Paulo (Alesp) defined, last February 8, at a meeting held with the FOAESP’s president, Rodrigo Pinheiro, the agenda for the first semester. According to the Alesp agenda, the public hearing to discuss the funding for Policy Actions against HIV/AIDS through the Ministry of Health’s Ordinance 3992/2017 is scheduled to April or May. Among the Organizations, the State Council of Municipal Health Secretaries (Cosems/SP), the Secretariat of Health of São Paulo (SES-SP) and the FOAESP will be invited. Besides the Organized Civil Society, it is hoped to mobilize the State STD/AIDS and Viral Hepatitis Coordinations.

Advocacy das hepatites entra definitivamente na agenda do FOAESP


O advocacy das ações das hepatites virais (HV) entrou definitivamente na agenda do Fórum das ONG/Aids do Estado de São Paulo (Foaesp). No final de janeiro, em reunião anual de planejamento, as ONG associadas ao Foaesp fizeram constar do documento norteador ponto específico para as ações de controle social e advocacy das hepatites virais em 2018.

Durante a reunião, a coordenadora do Programa Estadual de Hepatites Virais (PEHV-SP) da Secretaria de Estado da Saúde de São Paulo (SES-SP), Sirlene Caminada, apresentou dados atualizados sobre as HV no Estado. São coinfectadas com hepatite C 14,11% e por hepatite B 9% das pessoas com HIV em seguimento no Estado.

Entre outros pontos, Caminada sublinhou a necessidade de ampliar o diagnóstico das hepatites no pré-natal e estimular a vacinação da hepatite B especialmente entre populações vulneráveis. Ainda segundo a coordenadora do PEHV-SP, é necessário mudar a logística de tratamento de modo a democratizar e ampliar o acesso, independente do grau de fibrose, visando a garantir o fornecimento de medicamentos para a hepatite C. E nos serviços, as ações de prevenção devem incentivar a testagem, o confirmatório do diagnóstico com PCR, o tratamento da coinfecção e inserir o conceito de cura após tratamento adequado.

No Documento Norteador das Ações Políticas do Foaesp foram elencadas as seguintes prioridades:

- Estimular a ampliação da cobertura vacinal das hepatites A e B e a testagem em hepatite C, principalmente para as pessoas acima de 40 anos.
- Propor e acompanhar a criação da Câmara Técnica de Hepatites Virais no estado.
- Diante do surto de hepatite A no estado, há a necessidade de ampliar informações e encaminhamentos, sobretudo junto a populações vulneráveis ou mais afetadas (como HSH e outras).
- Garantir a testagem continuada da hepatite C entre as PVHA em seguimento, conforme definido em protocolo.

Financiamento das ações
A Frente Parlamentar de Enfrentamento às DST/HIV/AIDS da Assembleia Legislativa de São Paulo (Alesp) definiu na quinta-feira (08/02), em reunião com o presidente do Foaesp, Rodrigo Pinheiro, a agenda para o primeiro semestre. A depender da pauta da Alesp, foi programada para abril ou maio, Audiência Pública para discutir o financiamento das políticas de enfrentamento do HIV/AIDS e das hepatites virais no Estado a partir da Portaria 3992/2017, do Ministério da Saúde. Serão convidados o Conselho de Secretários Municipais de Saúde do Estado de São Paulo (Cosems/SP), a Secretaria de Estado da Saúde de São Paulo (SES-SP) e o Foaesp. Além da Sociedade Civil Organizada, espera-se mobilizar as coordenações de DST/Aids e hepatites virais no Estado.

Health Sector studies to extend vaccination against Hepatitis A to specific groups

The Ministry of Health is studying to extend vaccination against Hepatitis A for men who have sex with men (MSM). The Department on STD, AIDS and Viral Hepatitis (DIAHV) is discussing with the National Immunization Programme (PNI) the possibility of offering HAV vaccine for “specific groups”.

The information comes from the Ministry of Health itself, as a response for the FOAESP – State of São Paulo Forum on NGO/AIDS petition, which asked the sector to extend vaccination in a letter sent in December, last year. In the answer, signed by the substitute secretary of the Health Surveillance Secretariat, the Ministry informs that from 2017 on, it is extended the HAV vaccine for “every child aged less than five years old”. It also lists the groups which vaccination is indicated to people no matter the age and highlights that “the DIAHV is taking a joint decision with the PNI about the possibility of extending vaccination to specific communities”.

The Ministry of Health document also informs that since 2016 the Hepatitis B vaccine “is available at the public service for all the population, independently of their age (universal campaign)”.

Eliminating Viral Hepatitis
In July, the Minister of Health Ricardo Barros affirmed that from this year on, every Hepatitis-C positive individual must receive – independently of the liver fibrosis score – treatment to combat the disease. Last January 12, the Technical Advisory Committee on Viral Hepatitis held a meeting in Brasília in order to update the Clinical Protocol and Therapeutic Guidelines (PCDT) for Hepatitis C.

The committee also evaluated the possibility of including two new drugs on the treatment of the disease. The committee final document will be presented in February at the National Committee for Technology Incorporation of the Public Health System (SUS) – CONITEC. If approved by the committee, the PCDT will be submitted to public consultation in order to hear the civil society opinion.

At the meeting, the director of DIAHV, Adele Benzaken, introduced the Eliminating Viral Hepatitis Framework, as well as the strategies recommended by World Health Organization (WHO), which set reduction of new infections by 90% and mortality by 65%.

Among the strategies frameworked are the extension of diagnosis and the treatment, as well as the evaluation of the epidemiological situation and the programme on eliminating viral hepatitis. According to the DIAHV, the Hepatitis C affects about 700 thousand individuals in Brazil and it is linked to most of deaths caused by Hepatitis and most of the liver transplants in the country.

The Brazilian Eliminating Hepatitis Strategy follows the WHO Protocol. “This initiative has its greatest challenge at expanding diagnosis and treatment for all population, as the improvement of the epidemiological surveillance to achieve the elimination goal”, affirmed the DIAHV director.

Saúde estuda ampliar vacinação contra Hepatite A para grupos específicos


O Ministério da Saúde estuda ampliar a vacinação contra a Hepatite A para homens que fazem sexo com homens (HSH). O Departamento de IST, Aids e Hepatites Virais (DIAHV) está dialogando com o Programa Nacional de Imunização (PNI) sobre a possibilidade de fornecer a vacina para “grupos específicos”.

A informação é do próprio Ministério da Saúde, em resposta ao pleito do Fórum das ONG/Aids do Estado de São Paulo, que demandou da pasta a ampliação da vacinação em carta enviada em dezembro do ano passado. Na resposta, assinada pelo secretário substituto da Secretaria de Vigilância em Saúde, o Ministério informa que em 2017 ampliou a indicação da vacina da Hepatite A “para todas as crianças com até cinco anos incompletos”. Também relaciona os grupos em que a vacina é indicada para pessoas em qualquer faixa etária e salienta que “o DIAHV discute junto ao PNI a possibilidade de ampliar a recomendação dessa vacina para grupos populacionais específicos”.

O documento do Ministério da Saúde informa, ainda, que desde 2016 a vacina da Hepatite B “está disponível gratuitamente para indivíduos de qualquer idade (vacinação universal)” para toda a população.

Para eliminar as hepatites virais
Em julho, o Ministro da Saúde Ricardo Barros afirmou que a partir deste ano todas as pessoas com Hepatite C devem receber – independentemente do grau de lesão hepática – os medicamentos que curam a doença. No último dia 12 de janeiro, o Comitê Técnico Assessor para o controle das hepatites virais reuniu-se em Brasília para atualizar o Protocolo Clínico de Diretrizes Terapêuticas (PCDT) de Hepatite C.

O comitê também analisou a possibilidade de incluir dois novos medicamentos para o agravo. O documento final do comitê será apresentado em fevereiro à Comissão Nacional de Incorporação de Tecnologias no SUS (Conitec). Se aprovado pela comissão, o PCDT será submetido à consulta pública para manifestação da sociedade civil.

Na reunião, a diretora do DIAHV, Adele Benzaken, apresentou o Plano de Eliminação das Hepatites Virais, bem como as estratégias preconizadas pela Organização Mundial da Saúde (OMS), que estabelecem a redução de novas infecções em 90% e de redução de 65% da mortalidade.

Entre as estratégias estabelecidas estão a ampliação do diagnóstico e do tratamento, bem como a avaliação da situação epidemiológica e a sustentabilidade do programa que visa a eliminação das hepatites virais. Segundo o DIAHV, a Hepatite C afeta cerca de 700 mil pessoas no Brasil, representa a maioria dos óbitos por hepatites e a maior parte do número de transplante hepático no País.

O plano brasileiro de eliminação das hepatites segue as recomendações da OMS. “Essa iniciativa, tem como maior desafio a ampliação do diagnóstico e do tratamento para todos, bem como a melhoria da vigilância epidemiológica para alcançarmos a meta de eliminação”, afirmou a diretora do DIAHV.

PLHA are going to undergo Hepatitis C tests once a year in Ribeirão Preto


From 2018 on, all people living with HIV/AIDS (PLHA) are going to undergo Hepatitis C tests at least once year in the city of Ribeirão Preto, located 315 kilometers northwest of São Paulo’s state capital. From 2015 to 2017, among the 1407 PLHA detected in the municipality, about 1018 eventually underwent HCV test during the treatment.

That was one of the main deliberated referral at the meeting held by the social movement and the Municipal Programme on STD/AIDS, Tuberculosis and Viral Hepatitis, held during the morning of December 27.

The problem, according to the coordination of the Municipal Programme, is that most people in follow-up no longer were tested during the treatment, that is a protocol  that will be retaken next year on. The information about people undergoing or that already underwent HCV treatment is also a problem, since in order to access the data, the coordination of the Programme needs to ask permission first.

However, according to Liz Neves, coordinator of the Municipal Programme, the sector is making further efforts on undertaking tests and treating the HCV coinfected population. Besides that, this past year, the supply of HCV testing has increased. In 2017 alone, it was identified 431 newly infected individuals in the municipality. Those people, however, have not already undergo PCR test (confirmatory testing), although they are being called to return to the healthcare system.

Hepatitis A
In 2017 it was detected 28 newly cases of hepatitis A in Ribeirão Preto (the 13 newly cases disclosed before, was related to 2017 first semester database). The majority of cases were  identified among men who have sex with men (MSM). The hepatitis A immunization is available only to youngsters under 13 and the population at greater risk of infection. The MSM are not among the prioritized groups. The State of São Paulo Forum on NGO/AIDS requested, last November, the expansion of vaccination for this group.
